I. Subject of the order: The subject of the order is the delivery of equipment for the needs of the Provincial Hospital named after Zofia Zamoyskich Tarnowska in Tarnobrzeg, divided into 10 packages.
II. Description of the subject of the order:

1) Purchase of medical equipment, including:

Package 2 – Endoscopic diathermy
Package 3 – Equipment for the Histopathology Laboratory
Package 4 – Clinical microscope
Package 5 – Middle ear analyzer
Package 6 – Set of tools for esophagoscopy and rectoscopy procedures
Package 7 – Morcelator
Package 8 – Device for the sonication of implants
Package 9 – Set of Instrumentation for removing the cup and stem of the prosthesis
Package 10 – Device for measuring IL-6 protein
2) Delivery of the aforementioned equipment;
3) Assembly/installation, commissioning, conducting tests and checks of the aforementioned equipment;
4) Training of designated employees of the Ordering Party in the operation of the aforementioned equipment;
5) Training of technical staff in the technical service and maintenance of the aforementioned equipment;
6) Warranty service for the aforementioned equipment, under the terms specified in § 81;
7) Delivery of the warranty card for the aforementioned equipment;
8) Delivery of the technical passport for the aforementioned equipment;
9) Delivery of the user manual for the medical equipment in paper form in Polish, in electronic form in Polish and technical (service) documentation.
5. Medical products that are the subject of this agreement must comply with the requirements of the current state of law, particularly those introduced by the Act of April 7, 2022 on medical products (Journal of Laws of 2024, item 1620, as amended).

The parameters indicated by the Ordering Party represent the technical, operational, usability, functional conditions and other features important for the subject of the order, while the specified brand or trade name defines the class of the product, not a specific manufacturer. The contractor may offer equivalent products, provided they have no worse technical parameters than those described and provided in the SWZ.
7. The contractor is responsible for the quality, compliance with the technical and quality conditions described for the subject of the order.
