Participation conditions: - Knowledge and experience: The Contractor will meet the condition relating to knowledge and experience if it demonstrates that: in the last 5 years prior to the deadline for submission of offers, or if the period of business activity is shorter, during that period, it has properly completed at least 2 orders each with a net value of at least PLN 1,000,000, whose subject were multi-module web applications including at least the modules: orders, invoicing, warehouse, role and user administration, integrated with KSeF. The assessment of fulfilling the condition will be based on the formula: "meets – does not meet", based on the list of executed orders submitted by the Contractor, providing: dates, software functionalities with regard to functionalities indicated in section IV.2.1) of the Inquiry, order value and the entity for which the order was executed (Attachment No. 3). - Technical potential: Has personnel who will be assigned to execute the order and who possess the following experience, education, and professional qualifications: 1) Business Analyst a) higher education, b) at least 5 years of experience in IT consulting, c) must demonstrate participation in implemented projects including at least the following modules: Warehouse, Production, Management in multi-module CRM/ERP/WMS class systems and show ability to work on mockups/prototypes, at least Figma or equivalent, d) participation as a Business Analyst in at least 2 completed IT projects involving delivery, development, and implementation of CRM/ERP/WMS class IT systems in companies with a total amount of at least PLN 300,000 net each, e) presence at the Ordering Party's premises minimum 2 days a week during the business analysis phase and minimum 4 days per month during subsequent phases of implementation, testing, and training. 3) Programmer a) higher technical education, b) at least 2 years of experience in IT project implementation, c) knowledge and experience in developing multi-module web applications including at least modules: orders, invoicing, warehouse, role and user administration, integrated with KSeF, d) participation in at least 2 completed IT projects involving delivery, development, and implementation of IT systems in companies with a total amount of at least PLN 300,000 net each. The Contractor must make available persons capable of executing the order as described above. Additionally: a) The Offeror has an official AI usage strategy in software production approved by the internal security/compliance department. b) The Offeror will provide application maintenance services with a high Service Level Agreement (SLA) at a minimum of 99%, and a 4-hour response time for critical errors. Failing to meet conditions (a) and (b) will result in rejection of the offer. Confirmation at the offer stage is made by indicating the appropriate answer in Attachment 4. The Ordering Party allows the Contractor to designate the same person for more than one position provided the conditions for each position are met. Professional experience of individual persons will be calculated in months so that 1 year means 12 months. Required periods of professional experience must be counted up to the deadline for submission of offers. Assessment of fulfilling conditions will be based on the formula: "meets – does not meet", based on the: • statement of fulfillment of participation conditions (Attachment 1) • list of persons assigned to perform the order (Attachment 3) together with information on their qualifications, experience, and education, separately for each person; • photocopy of documents confirming education (diploma), qualifications or completion of required courses. The Ordering Party reserves the right to verify the factual state against the submitted statements in detail, including by requesting documents confirming the fulfillment of the above conditions, e.g., CVs of specific persons, acceptance protocols or invoices concerning implemented projects. - List of required documents/declarations: According to the invitation.
