REQUEST FOR QUOTATION FOR THE DELIVERY AND INSTALLATION OF A PV SYSTEM 1. The subject of the contract is the design and installation of a photovoltaic micro-installation with an energy storage system using solar energy to generate electricity on the office building located at Czechowicka Street 43, in Bielsko-Biała. The planned photovoltaic installation has a capacity of 20 kWp and an energy storage capacity of 24 kWh. 2. Scope of works: a) Design and construction of the photovoltaic installation; b) Preparation of executive documentation; c) Delivery and installation of a complete photovoltaic installation with an energy storage system and connection to the building's internal electrical network at the Main Distribution Board. 3. The Contracting Authority, as part of preparation for the photovoltaic installation, has executed technical infrastructure including external DVR conduits with a diameter of 110 mm, located under hardened surfaces. Devices such as inverters and energy storages are to be installed in an external technical cabinet located at the transformer station. Power cables should be routed exclusively through existing 110 mm DVR conduits under hardened areas and along newly established routes on the building walls following the attached plan to the main distribution boards in the office building. 4. Detailed scope of work and description of the contract subject are included in Annex 1 to the Request for Quotation. 5. The Contractor is obliged to design and implement the photovoltaic installation adjusted to the actual technical and spatial conditions existing on the property. 6. The construction manager appointed by the Contractor will supervise all installation works specified in this request. 7. Where references to trademarks, patents, origin, source, or specific processes characterizing products or services of certain contractors appear in the contract description, the Contracting Authority allows equivalent solutions, provided the minimum requirements meet or exceed the parameters specified in the documentation. Likewise, references to standards, national regulations, approvals, and technical specifications allow equivalent solutions. If the Contractor offers equivalent solutions, they must indicate and prove in their offer that these solutions meet the requirements to the same extent as those specified in the request. 8. Cost optimization 1) The Contracting Authority allows the use of alternative technologies and materials provided they ensure at least equivalent functional parameters, durability, and compliance with applicable standards and legal regulations. Any change requires the Contracting Authority's approval, and the Contractor must provide appropriate documentation confirming compliance. 9. Participation conditions: - Additional participation conditions: 1) The Contracting Authority requires the Contractor to conduct a site visit before submitting an offer to obtain necessary information for the proper preparation of the offer and accurate pricing, as well as to confirm the proper execution of the existing conduit infrastructure and verify the possibility of its use without the need for additional earthworks, which – due to the already paved area with paving stones – will not be allowed. 2) The Contractor should arrange the site visit date with the contact person indicated in Chapter XIII of this Request for Quotation. 3) A protocol of the site visit signed by both parties will confirm the visit (protocol template is Annex 5 to the Request for Quotation). Preparation of the site visit protocol is the Contractor’s responsibility, based on the provided template. 4) The cost of the site visit (travel, use of own employees, etc.) and responsibility for interpretation of obtained information and other consequences of the site visit lie solely with the Contractor. 5) Failure to participate in the site visit results in the rejection of the offer.
1. Awarding the contract is conducted in accordance with the principles of competition and equal treatment of Contractors, as understood by the Guidelines on the eligibility of expenditures. 2. Entities personally or financially related to the Contracting Authority are excluded from participation. Personal or financial relations include: a) participation in a civil partnership or personal company as a partner or holder of at least 10% shares or stocks (unless a lower threshold is stipulated by law), serving as a member of a supervisory or management body, proxy, attorney-in-fact, b) being married, related by kinship or affinity in a direct line, kinship or affinity up to the second degree in the collateral line, or related by adoption, custody or guardianship, or in a common household with the Contractor, their legal substitute, or members of management or supervisory bodies of Contractors applying for the contract, c) being in such a legal or factual relationship with a Contractor that raises justified doubts concerning impartiality or independence regarding the contract awarding procedure. 3. Participation conditions: 1) Only Contractors meeting participation conditions may submit offers for the contract covered by this Request, i.e.: a) The Contractor must demonstrate that they have or will have persons: i. Construction manager – 1 person holding: • building qualifications for supervising works in installation specialization regarding networks, electrical and power engineering installations and devices without restrictions, • a member of the relevant professional self-government chamber, in accordance with the Construction Law and the Act on professional self-governments of architects, civil engineers, and urban planners,
ii. Installer – at least 1 person holding: valid SEP G1 qualification certificates for operation (E) and supervision (D) Building qualifications are understood by the Contracting Authority as those issued in accordance with the Construction Law of July 7, 1994 or equivalent valid titles issued according to prior regulations or valid titles granted to citizens of the European Economic Area and the Swiss Confederation, subject to Art. 12a and other provisions of the Construction Law and the Act of December 22, 2015 on recognition of professional qualifications acquired in EU Member States. This criterion will be verified on the basis of a list that is part of the offer form. b) The Contractor should be in a good economic and financial situation: The Contractor meets the condition if they demonstrate having a paid liability insurance policy related to the contract for a guarantee sum not less than PLN 100,000 valid on the date of submitting offers. This criterion will be verified based on a copy of the paid insurance policy attached to the offer, valid on the offer submission date.
IX. Bid bond 1. The offer must be secured by a bid bond of PLN 2,000. The bid bond must be provided before the deadline for submitting offers. 2. The Contractor shall pay the bid bond via bank transfer to the Contracting Authority's bank account no.: 08 1020 1068 0000 1602 0531 2907 at PKO BP S.A. with the payment title: “Bid Bond – Photovoltaic installations – office building”. The date of bid bond payment is the day of crediting the Contracting Authority's account; for the offer to be valid, the crediting must occur before the offer submission deadline. - List of required documents/declarations: 1. Completed and signed offer form with the following documents and statements: a) Price list; b) Product catalog cards in Polish, installation and operation manual in Polish, CE declaration of conformity and warranty; c) Declaration of no personal or capital ties with the Contracting Authority – according to Annex 3 to the Request for Quotation; d) Contractor's statements on: 5 qualifications for activities covered by this contract if mandated by law; 5 having necessary knowledge and experience and possessing technical and human resources to execute the contract; 5 being in an economic and financial situation ensuring contract performance; which are part of the offer form; e) Contractor's declaration on fulfilling green public procurement conditions related to environmental protection – part of the offer form; f) List of persons assigned to perform the contract – part of the offer form; g) Scan of paid liability insurance policy; h) Power of attorney – if the offer is submitted by a proxy; i) Protocol from the site visit (copy). 2. Offers must be submitted electronically. The Contractor's offer and attached statements and documents must be signed with a qualified electronic signature or trusted signature by persons authorized to represent the Contractor according to the relevant register or record or based on a given power of attorney. 3. If the offer or copies of documents certified as true copies are signed by persons not listed in the registration document (record) of the tenderer, a corresponding power of attorney must be attached. 4. The power of attorney to submit the offer must be attached in the original or as a digital reproduction of the document bearing a qualified electronic signature or trusted signature of persons authorized to represent the Contractor according to the representation in the register or record.
